The Form: CLK/RA 3

Maine covers every entity type with a single filing called the Statement of Appointment or Change of Clerk or Registered Agent. On the state's forms list it is Form CLK/RA 3 (listed as CLKRA-3). One form for LLCs, corporations, nonprofits, and foreign entities alike.

Fees split by entity type under 5 M.R.S. § 103:

• For-profit entities, including every LLC: $35
• Nonprofit corporations formed under Title 13-B: $15

Watch out for older guides quoting $15 for an LLC. That is the nonprofit rate. The form header itself prints both numbers.

There is no online filing for this one. The form goes by mail to the Maine Secretary of State, Division of Corporations, UCC and Commissions. Expedited handling is available for $50 (next-day) or $100 (same-day).
